Suzette May, AGPCNP, B-C

I am an Adult-Gerontology Nurse Practitioner (AGNP), and registered nurse in Florida with over 10 years of nursing experience, including over five years in case management and transitional care nursing, specifically with Medicare advantage patients over the age of 65. I have a background in hospice, home health, long-term care, and wound care.

I earned my Bachelor’s in Nursing degree from Chamberlain University and my Master’s in Nursing degree from Purdue university.

I am dedicated to providing high-quality, comprehensive care to homebound and complex care patients, particularly those with chronic conditions, disabilities, and post hospitalization needs. My professional goal is to advance primary care, practice by improving patient outcomes and care continuity, with a focus on chronic disease management, preventative care, and health promotion.

Outside of work, I enjoy spending time with family and friends, appreciating the balance they bring to my life.
